As of May 2014 the New York Department of State has discontinued distributing biennial statement forms in paper format. Future biennial notifications will be emailed to filers who have provided the New York Department of State with an email address. If you would like to sign up to receive biennial statement filing notices via email please visit www.email.ebiennial.dos.ny.gov to provide an email address to the New York Department of State so they can email notification to you when your biennial statement it is due.
